How to Enroll in the Center and Make an Appointment:
1. Read, complete the information and sign all required forms. Students 18+ years of age, may complete the paperwork.
- Download, print and complete the patient form OR
- Complete and sign using the *new* online form function OR
- Call 970.488.4900 to ask a staff member to email them to you.

Return all completed health forms to the Health & Wellness Centers, preferably at least two (2) business days prior to your student’s appointment, by:

Use our *NEW* Online Form – doesn’t require returning by fax, e-mail, in-person or by postal mail. To plan your time accordingly, please be advised that completing the required form typically takes about 20 minutes.
2) Call 970.488.4900 to schedule an appointment. Please, no walk-ins.
3) Arrive 15 minutes before your scheduled appointment time, so students can complete their Health Needs Assessment.
Please note: while we always welcome parents, we do not require a parent to be present. We are able to provide services to your student as long as the required health forms have been completed, signed and returned to the Center.
